We are in network with Aetna, Anthem Blue Cross, Cigna, Optum/UnitedHealthcare, and Blue Shield of California plans only. To confirm that your specific plan is eligible and in network, please complete our contact form so we can verify benefits and guide you on next steps.

Even when we are in network, your out-of-pocket cost can still vary based on your plan’s copay, coinsurance, and deductible. If your plan requires you to meet a deductible, you may be responsible for the in-network allowed amount (the contracted rate we’ve agreed to with your insurance) until your deductible is met. This is typically different from our self-pay (cash) rate.

If I am not in your network and you choose to request reimbursement from your insurance company, you will be provided a receipt or “super bill” to submit to your health plan. Keep in mind that a request for reimbursement does not guarantee payment by insurance companies and insurance companies may choose to use the health data they have collected about you to decide future coverage eligibility and premiums.

OUT OF NETWORK CASH RATES

• 60-90 minute initial consultation: $600

• 50 minute therapy and/or medication management: $400

• 25 minute follow-up: $200

• Ketamine infusion: $600 each (6 infusions recommended for initial course)

• Some evaluations require more than one session. Additional sessions are in 50-minute increments and will be billed at the 50-minute rate.

• Payment is due at the time of service.

  • Initial session with mapping: $500

  • Subsequent sessions: $300

  • Total package of 36 sessions: $11,000
